Stepping up for Vulnerable Children

Did you know that only half of the foster families in Salt Lake and Tooele counties can document their immunization status? That means that Utah’s Division of Child & Family Services (DCFS) is having a tough time placing younger and unimmunized children into otherwise available homes.

As one of these families, you’ve been selected by your DCFS Resource Family Consultant (RFC) or Foster/Adoptive Consultant at UFC to take part in a project intended to make more families available for placements. As part of Project: ShotsUtah Foster Care is partnering with the Utah Department of Health to inform prospective foster parents as to which immunizations are listed in their database.

IT ALL STARTS WITH A SIMPLE FORM…

Please have each individual (aged 18 and older) in the home for whom you do not have immunization documentation fill out and submit the form below. Once completed, a representative from UFC will look into the Utah Statewide Immunization Information System (USIIS) to assess which immunizations are still required. You will then be emailed a PDF file of the immunizations on record.

What do you do next? With this record in hand, you can simply set up an appointment at any pharmacy, present your immunization record and insurance card to the pharmacist. Most insurance companies will cover immunizations at no cost. However, please check with your personal insurance to confirm coverage.

Once the initial round of vaccines has been administered, you will be considered current on your immunizations and will be able to accept placement of children in foster care for whom there is no immunization history! Remember…be sure to stay up on your boosters to ensure continued compliance!
