Deseret Industries (DI) has once again made a generous donation of vouchers for children in foster care.

These vouchers can only be used to purchase used clothing and donated “as-is” merchandise, as well as new value items limited to socks, underwear, towels, sheets, blankets, white shirts, men’s suits, and coats. New mattresses may not be purchased with DI Vouchers.

Due to changes in the voucher program the DI provides, there are new protocols that have been set in order to distribute the vouchers.

Foster parents will make a request online for DI Vouchers at https://utahfostercare.org/resources/you will need to know the following information when you fill out the request form:

Name of the primary care provider
Address of the foster parent
Telephone number of the foster parent
The DCFS region in which you live
Name of your Resource Family Consultant (RFC)
First name of child(ren) for whom you are requesting vouchers – you may request vouchers for 3 children on one form, if you need vouchers for more than 3 children, you will need to fill out another form.
Items needed by the child in your care. For example:

3 pair pants boy size 8
new winter coat boys size 8
desk
lamp

Once you have made your request, vouchers will be mailed to you on the following Friday. Please allow two weeks for delivery.

PLEASE NOTE:

The mailing for the vouchers will include a letter with the requested items listed for your convenience. The vouchers will be in a separate sealed envelope with an order form, you must take the SEALED envelope to any Deseret Industries store and present the sealed envelope to the Customer Service Leader (“CSL”) who will then assist you with your purchases. Any unused vouchers accompanying the order are retained by the store and voided. Vouchers have no cash value.

Vouchers will be valid through October 31, 2012. When you have a new child come into your home, you may request vouchers for them when they arrive.
