Please fill out the pre-approval form PRIOR to enrolling in classes. Once you’ve completed your classes, use the approval form (at the bottom of this page) to receive the pre-approved reimbursement.
HR | Tuition Reimbursement Pre-Approval
Criteria: 1) A "B" grade or better is required. 2) Six (6) months of employment with Utah Foster Care prior to the beginning of the course. 3) Commitment to Utah Foster Care (see Employee Handbook). Terminating employment prior to agreed time commitment will result in paying Utah Foster Care for tuition reimbursement related to the incomplete commitment. This does not constitute a contract or change the at-will employment status of Utah Foster Care. 4) Educational Reimbursement is limited to $2,000 per the fiscal year in which the course is completed. $10,000 maximum per employee.
