DCFS (Utahโ€™s Division of Child & Family Services) is concerned with the overall safety and well-being of each child in foster care. One way they show this commitment is to ensure that un-immunized children are placed into homes that can show that everyone in the home is current on immunizations.

Did you know that most foster families in Utah canโ€™t document their immunization status? This means that DCFS is having a tough time placing infants and other unimmunized children into otherwise available, loving homes. Fully immunized families are desperately needed in order to provide homes for children in care.

Required Vaccinations

*Required by DHHS

MMR

 

Persons born before 1957: 1 dose for females of child-bearing age, and all adults not born in the U.S.
Persons born after 1957: 2 doses, 1 month apart

Varicella

2 doses
*If you've had chicken pox, just let licensing know. No further documentation is needed.

Hepatitis A

2 doses, 1 month apart

Hepatitis B

3 doses: 2nd dose 1 month later; 3rd dose 6 months later

Td/Tdap

1 dose of Tdap and then a Td booster every 10 years

Influenza

(OPTIONAL)
Recommended annually for everyone age 6 months or older

How Do I Show Proof of Immunizations?

Check with Mom!

Remember that little card you received as a kid? Probably not…but Mom might! Many applicants have successfully found their immunization records this way.

Vaccine Titer

A vaccine Titer (pronounced tie-ter) is the measure, or level, of antibodies in the bloodstream. Your doctor can order this test. If you have any antibodies below a satisfactory level, you can get those specific immunizations redone.

Get Re-Immunized!

Most insurance companies will cover immunizations at no cost. However, please check with your insurance company to confirm coverage. 

Request your Record

USIIS is a confidential, web-based information system that contains immunization histories for many Utah residents. Participation in the USIIS system is voluntary and not all healthcare providers may participate or submit vaccine records to USIIS.

To request your own immunization record, you must be of legal age (over the age of 18). If you are requesting the record of a minor child (under age 18), you must be the childโ€™s parent or legal guardian at the time of the request.

What Do I Do From Here?

Once the initial round of vaccines have been administered, you will be considered current on your immunizations and will be able to accept placements for child in care without a vaccination history. Remember…be sure to stay current with required boosters once the required doses are complete, and ensure continued compliance.  Always check with your doctor if you have questions about your medical care.
